My PPTools toolbars and buttons don't work

Problem
You click on the PPTools toolbar buttons or use the PPTools menu item in Powerpoint but nothing happens.

Possible Causes
PPTools and other toolbars in PowerPoint can get "disconnected" from the add-in that created them in several ways. This can cause the toolbars to hang around even when the add-in isn't loaded. Here are a few things that might cause this and possible solutions:
